Position Tomball, TX – Permanent Palliative Care Nurse Practitioner
Rhino Medical is leading the charge seeking a compassionate and
motivated Nurse Practitioner (NP) to join a growing Palliative Care
program in the greater Houston metro area. This is a permanent,
full-time opportunity where you’ll deliver life-changing care for
patients in home health and long-term care (LTC) settings. Whether
you’re an experienced NP in palliative/geriatrics or a new graduate
with a strong RN background, this role offers the chance to
practice at the heart of patient-centered medicine while enjoying a
supportive, mission-driven team environment. Position Highlights
Location: Tomball, TX (Houston metro, 75-mile service radius)
Contract: Permanent, Full-Time (2-Year Commitment) Setting: Hybrid
Home Health LTC Facility Coverage Start Date: ASAP Schedule:
Monday–Friday | 40 hours per week | Flexible routing Patient Load:
Home Health: ~7 visits/day LTC Facility: Up to 12 visits/day
in-house What You’ll Do Conduct comprehensive in-person assessments
and develop individualized palliative care plans Manage symptoms,
provide emotional support, and guide patients through advanced care
planning Collaborate with interdisciplinary teams (nurses,
physicians, social workers) to ensure continuity of care Educate
patients and families on disease progression, treatment options,
and care preferences Document all encounters thoroughly for
compliance and billing What We’re Looking For Licensure: Active
Texas NP license unrestricted DEA Certifications: BLS required
(ACHPN or Palliative Care certification a plus) Experience: Open to
new grads with strong RN backgrounds in ICU, LTAC, or home health
Experienced NPs in palliative, hospice, geriatrics, or internal
medicine strongly encouraged to apply Skills: Independent,
empathetic, excellent communicator, organized, and confident with
