Job Description:
A company is looking for a Telephonic Care Navigator.
Key Responsibilities:
Completes telephonic outreach to members and educates them on care
gaps
Assists members with social determinants of health and connects
them to community resources
Maintains metrics and provides excellent customer service while
adhering to confidentiality regulations
Required Qualifications:
High School Diploma / GED or equivalent work experience
Must be 18 years of age or older
1+ years of call center and/or telephonic customer service
experience
1+ years of professional experience in an office setting using
telephone and computer
1+ years of healthcare, insurance, or community outreach
experience
